Ankylozujúca spondylitída (AS) je chronické zápalové reumatické ochorenie charakterizované bolesťou a štrukturálnymi zmenami. Pre ankylozujúcu spondylitídu sú charakteristické funkčné poruchy, ako je znížená pohyblivosť a axiálna deformita, ktoré vedú k zníženiu kvality života pacientov. Cieľom článku je priniesť základné informácie o terapeutických možnostiach cvičenia pri liečbe. Terapeutické cvičenie je najdôležitejšia nefarmakologická intervencia u pacientov. Okrem pozitívnych metabolických vplyvov zohráva cvičenie dôležitú úlohu aj pri zlepšení spinálnej mobility a svalovej sily. Udržanie správneho posturálneho držania javí kľúčovú úlohu u pacientov s ankylozujúcou spondylitídou. Kontrakcia svalu stimuluje a podporuje vylučovanie cytokínov s názvom myokíny. Najznámejší je interleukín-6 (IL-6). Tento myokín nepriamo pôsobí aj pri expresii tumor nekrotizujúceho faktora alfa (TNF‑ α). U pacientov vedie cvičenie k zlepšeniu kvality života, zlepšeniu fyzickej kondície a zníženiu bolesti. V industriálnom svete incidencia a prevalencia rakoviny pľúc narastá. Pri patogenéze zohráva významnú úlohu vplyv životného prostredia a genetika. Dôležitú časť liečby zahŕňa rehabilitácia pacientov. Rakovina pľúc vedie ku vzniku fyzického impairmentu u pacientov. Je spojená najmä so zníženou fyzickou funkciou pacientov a zníženou kvalitou života pacientov. Rehabilitácia pacientov napomáha stabilizovať priebeh ochorenia a vedie k väčšej viere u pacientov čeliť náročnému ochoreniu. Dôležitú časť rehabilitácie zahŕňa funkčné vyšetrenie pacienta. Na vyšetrenie je možné využiť štandardizované testy ako 6minútový chodecký test, „chair stand“ test, „timed up and go“ test, doplnené o vyšetrenie svalovej sily a hmoty. Medzi základné terapie rehabilitácie patrí cvičenie. Pri cvičení sa využívajú základné formy ako aeróbne cvičenie a silové cvičenie. Rehabilitácia pacientov s rakovinou pľúc je spojená so zlepšením symptómov únavy a zlepšením kvality života pacientov. Ďalším významným benefi tom je zlepšenie funkčnej schopnosti pacientov. Cieľ: Hlavným cieľom článku je zhrnúť rehabilitačné možnosti liečby u pacientov s rakovinou pľúc. An accumulation of cardiovascular risk factors combined with an unhealthy lifestyle has recently become more common in cancer patients and survivors. It has been recommended to integrate a comprehensive cardiac rehabilitation model called cardio-oncology rehabilitation to mitigate cardiovascular risk. Nevertheless, cardiac rehabilitation interventions limit barriers in low utilization, further exacerbated by the restrictions associated with the COVID-19 pandemic. Therefore, it is essential to integrate alternative interventions such as telehealth, which can overcome several barriers. This literature review was designed as a framework for developing and evaluating telehealth interventions and mobile applications for comprehensive cardio-oncology rehabilitation. We identify knowledge gaps and propose strategies to facilitate the development and integration of cardiooncology rehabilitation telehealth as an alternative approach to the standard of care for cancer patients and survivors. Despite the limited evidence, the pilot results from included studies support the feasibility and acceptability of telehealth and mobile technologies in cardio-oncology rehabilitation. This new area suggests that telehealth interventions are feasible and induce physiological and psychological benefits for cancer patients and survivors. There is an assumption that telehealth interventions and exercise may be an effective future alternative approach in supportive cancer care. Statistical analysis did not show significant differences between the MG and EG: ECM/BCM, MGPRE-POST -0.001 ± 0.10 vs. EGPRE-POST -0.03 ± 0.07, t = -1.121, p > 0.05, d = 0.16; PA, MGPRE-POST 0.18 ± 0.29 vs. EGPRE-POST -0.28 ± 0.31, t = 1.201, p > 0.05, d = 0.17; MM, MGPRE-POST 0.3 ± 1.2 vs. EGPRE-POST 0.4 ± 1.0, t = 0.081, p > 0.05, d = 0.01; FM%, MGPRE-POST -0.04 ± 1.5 vs. EGPRE-POST -0.2 ± 1.3, t = -0.307, p > 0.05, d = 0.04. Chronic fatigue syndrome is described as myalgic encephalopathy. Various multifactorial causes are implicated in pathogenesis. Typical symptoms include fatigue, sleep problems, muscle pain, headache, concentration problems. Fatigue in patients leads to a significant impairment of quality of life. Patients often have psychological symptoms. Therapeutic options in patients with chronic fatigue syndrome include exercise and cognitive-behavioural therapy. Optimal timing of the daily program and sleep management play an important role for patients. The most common types of exercise with potential effect in patients include aerobic exercise, graded exercise, strength exercise. Exercise in patients is associated with improvements in fatigue, cognitive abilities and depressive manifestations. Sub-optimal intensity of exercise dose may lead to worsening of symptoms. In addition to genetic factors, lifestyle factors and environmental risk factors also have a significant influence on the development of colorectal cancer. Physical activity and exercise play an important role in prevention. The link between the protective and therapeutic benefits of exercise and cancer can be explained by several mechanisms, including inflammation, metabolic dysregulation, the influence of myokines and the SPARC protein. Several types of exercise can be used in the treatment of colorectal cancer due to the improvement of function and symptoms, such as aerobic exercise, strength training and other rehabilitation methods. Colorectal cancer causes impairment physical function of patientsRehabilitation therapies that improve patients' physical function, reduce the prevalence of fatigue, and improve their quality of life are important treatment options for patients. Originally it was understood as the procedure dedicating to solve health-related problems prior to the recruitment or prior to the unwanted manifestation during a military operation. In parallel, the pre-placement examinations of the employees in the industry was introduced by W.A. Sawyer in 1942 [2]. Later on, in 1952, in the German journal, the term “preoperative” is used in relation to the preoperative breathing exercise in thoracic surgery [3]. In the beginning of the 1980’s the meaning of the “prehabilitation” changed and it is now defined as “the process of expanding patient’s functional and psychological capacity to reduce potential deleterious effects of a significant stressor, which is the surgical procedure itself” [4]. The Cambridge dictionary defines “prehabilitation” in medical context as “activities done by someone before they have a medical operation in order to improve their physical strength and help them to recover more quickly after the operation” [5]. According to the BJA Education journal, prehabilitation is “the practice of enhancing a patient’s functional capacity before surgery, with the aim of improving postoperative outcomes” [6]. The Collins dictionary defines “prehabilitation as “the preparation of patients prior to major surgical procedures to enhance general health and well-being with the outcome of decreasing morbidity and mortality” [7]. So, basically, all definitions of “prehabilitation” are currently, one way or the other linked to surgery. Prehabilitation and PPPM Here we want to emphasize and extend the meaning of “prehabilitation” in the context of predictive, preventive and personalized medicine as presented by EPMA [8,9] because we find it to be an excellent example of practical application of PPPM in the modern healthcare services. Prehabilitation can use all the tools and knowledge accumulated and published so far in the predictive, preventive and personalized medicine, including the research in suboptimal health state [10] of the particular patient who is to undergo a planned therapeutic/surgical procedure, outcome of which depends on the particular health status of the patient. This status, in our opinion, should be assessed with the help of available medical technologies in order to predict potential problems and avoiding them through using adequate preventive actions, being it personalized nutritional support based on the particular analysis of the patient’s blood, particular physical exercises to increase fitness, psychological support, disease/procedure-related education of the patient, as well as using health-supporting environmental factors in all potential medical fields such as surgery, organ transplantation, oncology, obstetrics, dentistry, neurodegenerative diseases, genetically determined diseases, pandemic management, and other fields of medical care. In the context of our previous plans for implementation of PPPM in the hospital structure [11], here we propose implementation of prehabilitation into the routine procedures of medical care in any hospital that wants to increase the quality of provided healthcare and bring predictive, preventive and personalized medicine to the patients who will, undoubtedly, benefit from such a change. We are aware that realization of this vision is not an 22 easy mission to be accomplished and the question of proof-of-concept as it was in 2017 [5] but we suggest international cooperation among all subjects who share the same attitude towards turning current healthcare into the future healthcare may speed up the realization. The network of EPMA institutional members provides a very good platform for such a cooperation. Conclusion and expected impacts The vision is long-term but at the very beginning, it is reasonable to focus on the patients at high risk for postoperative morbidity and mortality, such as older surgical patients who are frail [12, 13]. Nonetheless, in the later phases of implementation it may likely be of value, as several researchers already propose [14] , to offer prehabilitation for a much wider spectrum of surgical patients, and, as we believe, also for other patients in need of specific medical procedures. The baseline in each of multimodal approach of prehabilitation (exercise & physical activity, nutritional optimization, psychological wellbeing, education and other), is personalized intervention, given the possibility of prediction, thus preventing negative clinical outcomes. A rapid return to full physical function after major surgery and/or other intervening medical procedures, has the potential to improve outcomes for patients, thus allowing them to return home and resume normal activities earlier. Furthermore, accelerated recovery is expected to have economic benefits and potentially more efficient use of both, the available hospital beds, as well as the use of always-limited financial resources in healthcare [15]. COVID-19 coronavirus pandemic has affected several areas of human and public life. The implementation of the restrictions affects free movement, leading to limited physical activity (PA) level. The main aim of the study was to determine the level of PA of university students in Slovakia during COVID–19 pandemics. Material and Methods. The sample consisted of 3128 university students. Data was obtained from students from three universities with different specializations. A questionnaire about lifestyle with questions about physical activity was used. The questionnaire was distributed using online communication systems of universities. The online questionnaire was answered by 3128 students. Results. Analyzed data were obtained from 3128 students. The results indicate that 38% of all students exercise 3-4 times per week, while 41% exercise 1-2 times per week, and 21% do not exercise at all, or exercise 1-2 times per month. Found weak positive correlation (r=0,337) was between the exercise frequency and perception of well-being, and a very weak positive correlation (r=0,187) between the exercise frequency and stress intensity. A moderate positive correlation was found between the perception of health and life energy (r=0,579). A moderate positive correlation was observed between the perception of health and physical strength (r=0,579). Conclusions. COVID-19 restrictions have been associated with the reduction of physical activity of the students. To avoid the negative effects of a sedentary lifestyle, it is necessary to promote activity among university students.
